Consultant Pharmacist

Network HealthCare Pharmacy is a closed-door pharmacy seeking a motivated Consultant Pharmacist for our White Oaks, SC location. The ideal candidate must reside in Columbia, South Carolina or south surrounding area.

Position Overview

The Consultant Pharmacist provides consultation on all aspects of the provision of pharmacy services in the facility. In collaboration with facility staff, the Consultant Pharmacist helps to identify, communicate, address, and resolve concerns and issues related to the provision of pharmaceutical services. The Consultant Pharmacist performs a comprehensive review of each resident's medication regimen at least monthly. The medication regimen review (MRR) includes evaluating the resident's response to medication therapy to determine that the resident maintains the highest practicable level of functioning and preventing or minimizing adverse consequences related to medication therapy. Findings and recommendations are reported to the Director of Nursing and the attending physician, and if appropriate, the Medical Director and/or the Administrator.

Responsibilities

  1. Working with facility staff on the implementation of pharmaceutical services procedures including, evaluating the process of receiving and interpreting prescribers' orders; acquiring, receiving, storing, controlling, reconciling, compounding, dispensing, packaging, labeling, distributing, administering, monitoring responses to, and using and/or disposing of all medications, biologicals, and chemicals.
  2. Assisting in the identification and evaluation of medication-related issues, including the prevention and reporting of medication errors and the provisions and monitoring the use of medication-related devices.
  3. Working with the provider pharmacy to establish a system of records and receipt and disposition of all controlled medications to enable an accurate reconciliation, determining that drug records are in order and that an account of all controlled medications is maintained and periodically reconciled. Assisting in the accounting, destruction and reconciliation of unused controlled substances and non-controlled substances as required by state and federal law.
  4. Assisting facility in defining schedules for administering medications to maximize the effectiveness, maintain appropriate serum concentrations (e.g., antibiotics, pain medications), to avoid potential medication interactions, and, when possible, considering residents' choice.
  5. Assisting in the assessment and improvement in nursing staff medication administration, including infusion therapy and use of medication delivery and testing devices, through medication pass observation and through medication record reviews.
  6. Providing oversight and instruction on pharmacy regulatory compliance issues.
  7. Helping develop medication-related documentation procedures.
  8. Assisting in establishing quality assurance and continuous quality improvement (CQI) activities regarding the medication use process: prescribing; dispensing; storing; administering; and monitoring of medications in the facility.
  9. Serving on facility committees as required or required including Quality Assurance and Performance Improvement.
  10. Reviews the medication regimen of each resident at least monthly.

Qualifications

  1. A pharmacist who has graduated from an ACPE Accreditation School of Pharmacy with a Doctorate in Pharmacy.
  2. Should have a current resident pharmacist license and be in good standing.
  3. Proven experience as a clinical pharmacist.
  4. Solid knowledge of health & safety guidelines.
  5. Outstanding communication skills (verbal and written).
  6. Aptitude in problem-solving and decision-making.
  7. Strong professional ethics.

Solid, dependable, and verifiable work history required. Must pass background check and drug testing.
